not meet...肯定不对因为你的报告是过去式嘛,表头和下面你填写的内容应该组成一个结构完整的句子,一般的表达是evaluation meets the requirement,把它变为否定句,evaluation does not meet the requirements.
举个最简单的例子I like it--否定句的话I do not like it,你肯定不会说成是I not like it.
